BEREAVEMENT CLIENT INTAKE FORM
I lost my husband very unexpectedly. All our future dreams of starting a family and growing old together were crushed in the blink of an eye. This greatly traumatized me and my sense of loss and grief was overwhelming. My expressive art therapy sessions helped me work through some of these things. [The art leader] was very loving, caring, and supportive as I fell apart while creating each piece of art. She helped me find parts of myself that were lost in a sea of darkness.
